F/T position avail. Wilkinsburg. Supervisory exp. pref. Must be currently licensed RN with at least 2 yrs. exp. in nursing or psycho social assessments. Must have one yr. exp. in nursing or at least one yr. professional exp. & at least one yr. as an attendant care prog. consumer. Must have exp. work with consumers with TBI. Exp. with community based svc. delivery pref. Aptitude to use MS Word, Excel & HCSIS is reqd. Personal means of travel in svc. area reqd. Resp. incl.: provide svc. to consumers & staff rel. to supv. of attendant assisted basic & ancillary activities, train consumers, attendants & staff on health welfare & safety issues, assist with svc. delivery prob. solving related to health, welfare & safety issues, assist with consumer assessments to determ. eligibility for waivers, work with other provider agencies, dev. training mtls.& progs. to meet waiver stds. and all responsibilities per attached job description (click here).
